Diabetes Self-Management Education (DSME) 2022 Mini Grantees

The Health Planning Council of Southwest Florida, Inc., in collaboration with Big Bend Health Council and WellFlorida Council, Inc., is pleased to announce that the following agencies have been granted funding for the development or expansion of Diabetes Self-Management Education programs for 2022/23:

 

  • Barnabas Center, Inc.
  • Beyond Before Community Development Organization
  • Big Bend Area Health Education Center
  • Core Health Partners
  • DeSoto Memorial Hospital
  • Heart of Florida, Inc.
  • Holy Cross Health
  • Optimal Health Foundation
  • The YMCA of Florida’s First Coast
  • Treasure Coast Community Health

 

 

We received a great number of applications this year. We were pleased to see so much interest in the program and enjoyed reviewing all of the applications. It was very difficult to select the applications to fund with so many great programs across the state. We will be issuing another application for funding in the summer of 2023, and we hope that many of the agencies who were not funded this round will reapply. In the meantime, we will be sharing educational materials related to diabetes self-management education throughout the year.
